I have always played a C after the first Am.
In other words play C where the G is and squeeze the G in before the D...

Most of the other tabs/chord charts I've found show an Am7 instead of Am. If you listen closely, it does give that dissonant undertone that permeates the entire song.

I've seen hank live play the intro w a D chord then either Am or Am7 then just simply move the D chord down to the 5th fret and try ending it with the D string 3rd fret and pull off to the 2nd fret and climb up ending with a bend/pull off on the low E
