Song is played with capo on 1st fret,
but the chords of the verses are all played in the 9th, 10th and 11th fret. 

So that means that relatively to capo the chords are: 
 
C*: 0-10-9-0-8-0 
Em+*: 0-9-9-0-8-0  (i'll jut call it Em*)
F*: 0-10-10-9-8-0
G*: 10-9-0-0-8-10

The chorus is played with normal chords. 
Only one strange chordname:
CtoD: 0-5-4-0-3-0 (it's a C, but slided to 3rd fret, so it becomes a D)

You're supposted to pick the chords, which you can easily figure out yourself

Well you just have to pick the chords above. The pattern of that is like this.
