Overall, not a bad job. Not sure I agree with the Cadd9, as I don't hear a D being played. Also, in the chorus, both of you missed a chord transition in the last line. This isn't surprising, since it may not be easy to figure out. In this line:
Yeah [A/C#] don't tell em all [Bm] the truth, [E]
That's written as played without a capo. If you want to try that with a capo at the second fret, here's how to play it (treat the capo as position 0):
