The Bb/Eb chords don't sound right to me-- I think they are just Eb. And there should be an Eb (or Bb/Eb, if you prefer) after both of the Fm7 chords in the chorus. Overall, if one is going to go with my Eb premise, then the song is easily played with a capo on fret one, making the Intro/Verse chords: D-Bm-F#m7-Em7-D-F#. Thanks for mapping out the chords to such a beautiful song.
