Everything is right, just the first chord from the chorus, i think it's Dm insted of Bb. something like this:

I am not 100% sure, but to me, it sounds better this way. I hope i helped.
I am so ashamed, just listened to the song once again, noticed you were right, SO sorry.
I also think you're missing some chords in there somewhere,listen to the bass during the first verse. it changes quite a bit more than the chord changes you have written here. What you've got sounds right, it just sounds like it's a little lacking.
