Nice job! However, a few parts missing -- I don't mean the superfluous key change, I don't play that myself and anyway, it's not in the original. But the essential A major to start the second line of each verse is crucial, and only the first chorus ends "For we were young...", every chorus after that ends "Those were the days, oh yes those were the days". The instrumental part after the first chorus is correct, but for the choruses after that the instrumental is a repeat of the chorus chords.

Good work! The only thing I would add is on the second line of each verse (Where we used to raise...) I would start with an E7 instead of continuing the E chord.

Also, play the last B chord in each verse as a B7 ?
