This sounds good, a few adjustments might be good:

You might wanna throw in a G chord after the C in the chorus, before the D, right after "How-----" sounds nice.
Some of the lyrics are wrong,

sorry and true is actually " so real and true",

A hole in my sleeve,
Brown an' all broken.

Should be "My heart on my sleeve, the rhyming all broken", and

I tripped and landed should read "Tripped up and blinded".
