As with the a lot of Port of Morrow, I've worked this out in about 20 minutes just
playing and listening to the song. The rhythm is easy enough, and I've tried to line up the
chords as best I can with (what I believe to be) the lyrics. As always, works best
listening to the song! These should all be played in barre chord form. The F#m should definitely
be played up at the 9th fret, sounds much better that way! I wasn't really sure about
the instrumental break, with the sounds and bass over it.
Hope it all makes sense and helps you play - or at least attain a closer version
of - this song! :D
UPDATE: Watching it live, there are a few changes. A Bm in the chorus, and it
appears that James Mercer only plays Bm and E throughout the break.

A C#m x 7
I think the 2nd D in the verses should be B#m (79977)
Otherwise, sounds great!
Also, for the instrumental bridge:
B#m B#m (or A...) F#m E sounds pretty close...
(79977) is just Bm. But you're right. Just gave it a listen, definitely B bass notes! I will amend this immediately! For the bridge, it seems like the bass is playing B, A, E. Very good call on this! :D
