Some suggestions:
- according to my ears ;-) the bridge (I never made promises lightly, ... is actually Asus2 - D - G/B - D - Bm7 - D, and then indeed G/B - A - D for the 'we'll walk in fields of gold
- and the outro is: you end the last 'gold' on a D(no3) and continue immediately, so: D(no3) - D - Gadd5/B (in total 3 times) and end on a D(no3).
I love this song. Brides here in the UK always have Fields of Gold on their requests. Great chords.
