After the first two verses, you don't just play C and G - You play it like this:

Two corrections based on playing this on keyboard:
* Bm should probably Em/B (chord progression to get descending bass line)
* As @WillieMan mentioned, chorus does do C F G F, but with bass doing C, so it's C F/C and G with C (I forgot what guitar chord this'd be... G11?)
