Firstly, you've transcribed the song a whole tone too high.
Starts on a Gm and transpose accordingly.
Moreover, I'm not convinced the Em chord you have in both the verse and chorus is correct. Transposing down, you're suggesting it's a Dm in the verse. I believe it's a Cm. So the chords should read:
Gm/Cm/Eb/Bb/F
In the chorus it should be a Gm, instead of a (transposed down again) Dm.
So Bb/Gm/Eb/Bb/F
Other than that it's okay. Three stars.
Actually, I stand corrected. It IS a Dm in the verse.
