Not sure about the chorus but improvement suggestions would be much appreciated!
I think the verse chords, with decending bass line, are-
D - F#m/C# - A - C/E - Cm/G (twice)
G - Gm - A6 - A
For the choruses I play a first inversion D7, and to get the bouncing C to D note, keep holding the D7 chord and hammer on with the pinkie on the B string at the 3rd fret to make a D major. Not ideal, but kinda works.
In the coda I think it's just an F# major chord, not as melodic as your minor seventh, but I think it's right.
