WTF wats an Hm i think u might want Bm or something like that for the first one but for chorus i was fooling around and the chorus one should be F#7 (i think that's what you call it) i tabbed it below.

this is an add-on to my last correction....the Bb and the "Hm" should be switched around.
P.S. i played the F# chord and it sounds..ok not the greatest but better than Hm :P
