This is my first transcription, so bear with me. I figured these chords out for an 
as best as I could. I haven't figured out the 2 measure intro though, but if I do
I'll update this.

Also, on the B minor, I found it best to not play the high E, but it's subjective. 
the chords during the verse start just before the lyrics do. You can listen to the song to 
a feel for it, it's pretty straightforward.

During the bridge, play D-A-G-A repeating throughout, then the chorus rhythm starts 
and repeats until the end of the song.

I've figured out the same, but I would play the verses like this: D-Bm-A-G, just think about that!

I'd use Asus4 instead of just A.
It sounds quite a bit more accurate.
