But for the sake of theoretical music preciseness and learning purposes, the chords "G/D" and "A/D" should have their name changed and be called "Gadd2/D" and "Aadd4/D" respectively.

And you're right, of course, there is an "a" in the G/D chord and a "d" in the A/D, so technically speaking this should be reflected in their respective names. I mostly avoid the more sophisticated chord names, however, because UG does not recognise them as chords and even manual highlighting mostly does not work, either. I rather give the correct voicing. But I'll add the correct names to the chords-section at the beginning.
