what does d/add 4 mean?

D(add4)/B x-2-0-0-3-2

I don't know if that's the right name but it is a D with the major third (f#) and the 4 (g). Might be interpreted as a Gmajor 7th chord with B as its root, too. Here it is just a transitional chord anyway. Did this help?
