A number of problems here:Splitting the tune into Verse, Chorus and Bridge, here are the corrections:
Verse- basically accurate, except that the D chord is always a D Maj7 chord- while the D chord will at least not produce any wrong notes, the Major 7 is VERY important to the feel of the song.
Chorus: Again, no bad notes produced, but where you have G they are actually a Em7, and again all D chords should be D Maj7. Subtle differences, but critical to the feel of the song
Bridge: I'm afraid you got totally lost here the chords, including the short instrumantal interlude before the vocals come back in:
Cmaj7 / Dmaj7 / Cmaj7 / Dmaj7 / G F#m / Em / G F#m Em / A7 /
Cmaj7 / Dmaj7 /Cmaj7 / Dmaj7 / then back to the verse

Forgot this part in the chorus. Chords are:
/ Bm7 / F#m7 / Bm7 / F#m7 / Em G / Dmaj7 / Cmaj7 / Dmaj7/

and the end of the song repeats / Cmaj7 / Dmaj7 / and fades
