The bridge has something sliding to an E and 
ends on Cm and D.  So if someone else can figure

Actually, it's in Eb tuning (down half a step), so all the chords you wrote go up 1 whole step (D becomes E, A becomes B and so on). And you're supposed to leave to high E and B strings open in all the chords formations.
That's it, not bad.

the version i have on my computer sounds like its in standard, when i play these chords w/ it, it sounds good
