The "F" is an "F/C", which I actually wrote but it got changed some way when it was uploaded.

Pretty good, the D/C kinda works, but should really be Am6/C (x3424x)
The A/C# should be an Aaug/C# (x4322x), the F/Eb an F7/Eb (xx1211),the D/F# an Daug/F# (xx4332), and the Bb/Ab a Bb7/Ab (45444x)
The original key is F, so the capo should be at the 3rd fret, not the second, for this to match.
The missing lead breaks sequence is D - Ebdim - Em - Fdim/B - F#m - G - Gm
