Not that bad overall, but there are a few mistakes:

In the verse, the C should be Cm/Eb (or Eb6), as the first chords are played with a chromatic progression:

F    Am   Eb6   D7   
e--x----x----x-----x----|
B--6----5----4-----3----|
G--5----5----5-----5----|
D--7----7----5-----4----|
A--8----7----6-----5----|
E--x----x----x-----x----|


Also, in the chorus, the F and C should be Fmaj7 and Cmaj7.

Otherwise, I think it's ok.
