Hello, I know Dodie plays this on a baritone ukelele but it tunes to the same EBGD as the bottom four strings
of a guitar so I'm just going to go with guitar chord grids/layouts/whatever they are

She plays some chords in first and second inversions so for those I've put the bass note
after a '/'

If you transpose it up 5 steps you get the 'normal' ukulele chords :)
