Hey great job!! There's only two things I heard. In the verses: in second part of the first messure he moves his finger to the third fret of the A string. (hope that wasn't confusing!!)
So its like you pick the Em through once, then put your finger on the A string, third fret and play it through then change to the rest of the chords. (gosh I confused myself!)

and the other thing: the 4th chord that you were looking for is called a G/F# which is simply this:

i think a regular D (2320xx)`fits better....but whatever, it's all from perspective. good job!

wow, dude, the chords for the chorus are: A minor, C, F, and i think G.... the verse doesn't sound right at the end either.
