Good tab!

There's one correction I'd suggest for the verses/outro, though. I think, the main part goes like this:
| C#m | G#m7 | F#m7 | G#m7 |

For the chorus, I'd choose a different voicing for the Em7/D, making it more like a G/D:

E-A-D-G-B-e
G/D x-x-0-4-3-3
