You completely missed the chord that makes the song:

x 5 6 7 7 7

This chord is used after "make it all worthwhile". I suggest you watch the video from this performance. A few other chords are wrong too. Especially in the chorus.

Right, firstly, your D chord is either on the wrong strings, or you mean "2" instead of "3".

Secondly, I think the B chord may be wrong. Sounds bad when you play along.

Other than that I think you have the right chords, just not played in the correct way. In fact, if you listen to the live radio performance (it's on Youtube somewhere), I think the song may be in drop D, as you can hear him downtune before it begins.

The correct lyric is "the perfect cleft" not "the perfect glare". Makes much more since with the next line: "we all fall through."
