Should be F#m in all places and each line of the chorus resolves in G, it doesn't stay on D the whole time.

DrewDrew411 I agree, and on the chorus I like to throw an A in there before I go back to the D for the next line.
So each "come Lord Jesus come" line is D - G - A

D D2 Dsus4 D G A
Come Lord Jesus come

This is mostly a matter of personal preference, but it sounds better that way I think. You can also do a bass line walk up onto the G chord (E, F#, G)
