Nice chords. Only one error:

The quick G/D/G change at the end of the "I'm crawling on your shore" line should be G/C/D.

the Asus is wrong it should be 4th string 2nd fret not 3rd string and the D11 just try a C and slide it up 2 frets works perfect and is alot easier
