Ok so this isn't how Jack plays it but it is a starting point.
The video I looked at it was really hard to see what chords he was playing

Isnt he playing it tuned down to D?
Besides that it is pretty good, although the F chords after the Dm isnt played by Jack :)

If you want the actual intonation of the strings that he plays in the recording, transpose this down 3 half steps and put a capo on the 3rd fret. As a rule, when Jack plays live he tunes down a half-step, though in videos I've seen him play with the same chords here.
