Corrections are welcome. Make sure you capo the third fret or it just sounds balls. All 
are relative to the capo.

I feel the Em D thing is wrong. Em yes, D not...

Well, the song is is in Gm, so this isn't even close. The opening doesn't work like described either. (Agreeing with stuwoof, the D doesn't belong, should be an Em6 I believe (really Gm6).

Sorry, I missed the "capo III" at the top. But the D is still wrong. Try an Em6 instead:
