The correction just made was saying to substitute the C with the D/F#.
Otherwise, everything was correct! (:

Thanks I actually realized that is correct BUT it should be:

Em C D/F#

C D/F#
all that's going wrong
Em C D/F#
But you've been there all along

You hit the D/F# right before you switch to the Em...at least that is how it sounds on the CD.
