Capo on 4th;

The chords to the other parts of the song are, relative to the capo;

    C
"Im heading out west with my headphones on,"

I believe that second hammer-on starts off open, not the second fret.

e|--------------------------------------|
B|-----0h1-----1-----------1------------|
G|-----0-------0---0-----0-----0---0----|
D|-----0h2-----2p0---0h3-----2---0------|
A|--------------------------------------|
E|--3------x----------------------------|

Also, I think the end of "Lick #1" is open as well:

e|-1-0-------------|
B|-----3-1-0-------|
G|-----------2-0---|
D|---------------0-|
A|-----------------|
E|-----------------|
