Instead of the A on the prechorus and B on the bridge, try Am on the prechorus and Bm on the bridge :)

Minor recommendations: the C# in disease try Csus2 for two beats and C# for two beats, gives a nice subtle "flare" to the song. Also in the Bridge adding the D#m over the "You" (first three)

Bridge chords were incorrect as per the original track. Rest of the chords ok.
