This is just my version; it probably wouldn't be considered traditional jazz 
chords. I like to use the following picking pattern throughout the whole song:

(With D chord for example)

I played F#m7 as (x04222)

It's capo 5 if you want to play it in the same key as the mother from How I Met Your Mother. And I don't care if you like the show or not, this song is beautiful, and her rendition of it was as well. Here's links to her ukulele version:
