Verse 1
G Am7
Worthy, You are worthy
          G/B C
Much more worthy than I know
G Am7
I cannot imagine
         G/B C
Just how glorious You are
    D C
And I cannot begin to tell
    G/B C
How deep a love You bring
  D C
O Lord my ears have heard of You
    G/B C
But now my eyes have seen

Then using the C chord slide up 2 frets before
going in to the chorus.

       G
You’re worthy
       Am7
You’re worthy
       G/B C
You’re worthy
       G Am7
You’re worthy to be praised
   G/B C
Forever and a day.

Verse 2
G Am7
Glory, I give glory
       G/B C
To the One who saved my soul
G Am7
You found me and You freed me
         G/B C
From the shame that was my own
D C
And I cannot begin to tell
    G/B C
How merciful You’ve been
  D C
O Lord, my ears had heard of You
    G/B C
But now my eyes have seen.

Chorus

On the Cd they go back to the intro,
then chorus before going in to this
bit – probably kind optional.

G Am7 G/B C
We’ll sing an anthem of the highest praise
G Am7 G/B C
We’ll sound an anthem of Your glorious name

I've also seen the intro chords as Em, Em6, Em9, which is just a little bit easier to play and fundamentally accurate:

Em: 022000
Em6: 045000
Em9: 054000
