think the first chord should be x-7-5-0-0-0 and the second chord should be x-5-4-0-0-0. Other than that i think its all good! :)

also for the bridge i think its these 4 chords:
x-7-5-0-0-0
x-12-11-0-0-0
x-11-9-0-0-0
x-5-4-0-0-0

with hammer-ons for the x-11-9-0-0-0 chord

nice dude!
additional info. seems the 1st chord is an Em variation (shape). But is an F# in sound.
the A6 shape is more of a D chord voicing. so the the verse goes Em - D - Em - C
