I figured you wanted to play in the key of C so this is what I corrected for you. I play it in key of A so I just transposed for you.
This just a suggestion to how I think it should be played, but you can still play it like you have it you want.

C C6 C C6 C6= is AM+ a C note(3rd fret,5th strg)

It's hard to know exactly where the chords go when it is put at the end of each sentence.
