It's difficult to break this up into verse, chorus, bridge, etc. because the sections are all short and flow together easily in this ballad. Em#5/C is just a C, with an added finger on the 3rd fret of the e string. Try starting off without strumming (just play each chord) and then add in rhythm with strumming for the build and choruses.
