I'm not gonna claim this 100% accurate, it's just a start; and something I threw
together so I could just play this tune.

Update, 7-21-13: Changed two of the chords to Cm7 and G6. Thanks to Bongobob13 for his correction.

Another small addition might be the little run down on the intro to go with the hammer ons on the D, to take the part of the piano. So the full basis for the intro would look something like:
