very good,
but I think that the C chord in "I'm tired of waking up in tears" shuld be G/D#.
and the last D/F# in "But the more I try to move on, the more I feel alone" is D without different bass.
and all the Cm's can turn to Cm6.

wonderful job! It really does sound best with capo 3! :) however, the Cm, is simply an Am...so you might want to change that so that you don't confuse any new guitarists who aren't very advanced yet! :)
