(its a bit tricky hear at the end but stick to the strumming pattern and it will help.)

And I'll forget the world I knew
Ep Ep F F
  But I swear I won't forget you
      B B Ep Ep F F
Oh, if my voice could reach back through the past

The chords are pretty much correct, but I'd like to correct the Fm (before modulation) to F#. and same goes for F while playing in B - just change it to F#. I don't know what Ep stands for, but I play G# there.

Btw, I play it on piano, so if the chords sounds good on guitar then I'm sorry for correcting (I can't play guitar so I don't know how it sounds).

You have it partly right, but in the wrong key.

It's in B instead of A how you have it. So the main chord progression is a full step up. so...
B major to Gb major to Ab minor to E major (you have to use the full bar chord on guitar, because it's the E major above the B)
