Look at this clip of Paul Weller performing this song live:

You will see that he is playing the Am7 in the open position, not at the 5th fret. He is also not playing the Em7/B as you show in the chords above - he is just sliding the Em6/9 up an additional fret (to the 5th fret).

He is also not playing the Bm11 as you have above - he is just sliding up the Am7 shape up two frets (not sure if its a full Bm7, but I think it is!)

There's also a chord run from the Em to the C7 after the "waiting for a change" part of the verse (1min.30secs into the video)- it sounds as if he is sliding up the Em and finishing on a barred C7 at the 3rd fret, but as I can't see the fretboard on that part of the video I'm not 100% sure what's going on here!!

I've worked out the rundown - it's a C7 shape starting at the 5th fret, then sliding down 5th / 4th / 3rd then to open C7. I hope that makes sense!

Also the Bm7 I refer to above ISN'T a full Bm7 - its 024020 - so he isn't playing the full barre on the Bm7, just using his barre finger on the A string only.
