When Ringo played with Ben Harper and the Relentless 7 on Live from the Artists Den, Ben Harper played the verses D, G, D, A, I watched the video since the above did not sound right for that performance. Not sure of the chorus.

To my way of thinking about form this song is actually more Verse- pre-chorus-Chorus-Bridge w/alternating sections of 12 bars(2 6 bar sections)for the verse/chorus and 9 bars(2 4 bar sections plus extra bar at end) for the PC/B- what do you guys think? BTW- nice versions on YouTube including some live ones

There is also the "CONCERT for GEORGE" (YouTube) version done in D... (at least when I was playing Bass along with it.) Your notations are really close... Thank you, for that. And... I've noticed a LOT of musician/singers dropping the key in which they sing a particular tune... as their vocal-chords age.
