Great tab, just for the F#m I would use Em/F# (just add the F# bass note) to make it sound more like the piano. This becouse on the piano you just play an F# note and not the whole F#m chord.
And I would add an Am7 like this:

Hi, Thanks for the chords, i was writing it out for a student and getting very stuck. I still found a couple of chords iffy, so i checked out the smart arse pianists on youtube, who always get it right. Swines. I found the Em in the verses only, were Em9 and the C in verses only, were CMaj7. In the chorus i used D/F# instead of Em/f#. Hope that's helpful.

tips for easier chords, play it on capo 3 instead(and start on Dm) thanks for a great tab!
