Capo 3rd fret.

[Intro]

Em C9 G D/F#

[Verse 1]

 Em Bm C G D/F#
I want to go back, forget that it's over
 Em Bm C G D/F#
Painted in black, when you left me alone
 Em Bm C
I never knew, she was sleeping next door
 G D/F# Em Bm C
With the kid I grew up with, and I can't go back to her
 G D/F#
 Anymore, no more

[Chorus]

 Em Cadd9 G D/F#
You tell me you were happier with him, you want me to stay
 Em Cadd9 G D/F#
And you tell me that you needed time but you push me away
 Em Cadd9 G D/F#
But when you try to take me back, my heavy heart just breaks
                     Em Cadd9 G D/F#
No, I can't lift the weight

[Verse 2]

 Em Bm C G D/F#
Put you in the past, try to forget you cause it's over
 Em Bm C G D/F#
And every time you ask I'll pretend I'm okay
 Em Bm
 You're inside my head,
 C G
In the middle of the night when I don't feel right
               D/F# Em Bm C G D/F#
I dream I can hold you, and I can't go back to you anymore, yeah, yeah

[Chorus]

 Em Cadd9 G D/F#
You tell me you were happier with him, you want me to stay
 Em Cadd9 G D/F#
And you tell me that you needed time but you push me away
 Em Cadd9 G D/F#
But when you try to take me back, my heavy heart just breaks
                      Em Cadd9
But, I can't lift the weight,
                  G D/F#
and I can't lift the weight

[Verse 3]

 Em Bm
And I can't lift the weigh, no I can't life the weight
 C G D/F#
Yeah, you stand on my shoulders and my heart just breaks
 Em Bm
And I can't lift the weigh, no I can't life the weight
 C G D/F#
Yeah, you stand on my shoulders and my heart just breaks
 Em Bm
And I can't lift the weigh, no I can't life the weight
      C G D/F#
Yeah, you lower your standards and I raise the stakes
       Em Bm C G D/F#
And I can't lift the weigh, no I can't life the weight

[Chorus]

 Em Cadd9 G D/F#
You tell me you were happier with him, you want me to stay
 Em Cadd9 G D/F#
And you tell me that you needed time but you push me away
 Em Cadd9 G D/F#
But when you try to take me back, my heavy heart just breaks
                      Em Cadd9
But, I can't lift the weight,
                  G D/F#
and I can't lift the weight

 Em C G D/F#
You tell me you were happier with him, you want me to stay
 Em C G D/F#
And you tell me that you needed time but you push me away
 Em C G D/F#
But when you try to take me back, my heavy heart just breaks
                      Em C
But, I can't lift the weight,
                  G D/F#
 but I don't wanna wait

Good but not quite. The D/F# is actually a G/F# and the chorus is Em7 Cadd9 G G/F#. The whole chorus is played with fingers 3&4 on strings 1&2 in the 3rd fret, same place as the Cadd9.
Going by the sound and from watching him play it :)
