well that took forever, just listen to the song to get a strumming patteren i like use 
my hand to mute every 2nd strum to get the same well kinda the same effect as a piano :)

f#m7 - not fm#7. Also play the 1st string, 5th fret (a) with pinky.

after 'home sweet home' use 'f' shape A chord (5th fret)- walk melodic notes down : a, g, f#, e.
