I think the last C#m of the chorus might actually be an E ?!
Other than that I love it, thanks so much :)

Try it with a C#m/E on the last line of the Chorus. (I.e. I play piano, so on my left I still play the C#, but on my right I play the"C#m/G#" and the "C#m/E". Basically, I play the first 3 lines as "A, B/F, C#m/G#", then "A, B/F, C#m/E" on the 4th line). Hope that makes sense.
