Props to theClifton, his version sounded very close, but I think this is more accurate.
 
During the instrumental interlude sections and the "I want to give it all up" sections, I tabbed the chords 
as "Cm-Em" because I believe that's technically correct, but an open C instead of Cm sounds just as good.
