dude, some of the chords are right, but a lot of them don't go along with the song at all. I like the format that you chose to write the chords, but it's messed. I found a couple other sources that seem to be right, in the same format as yours, but different chords. These seem right, while yours is wrong. Take it off man, or just fix it.
