A E G D
And when I see the sign that points one way

The lot we used to pass by every day

You're not to blame

A E G D
From deep inside the tears that I'm forced to cry

For me, it cries

Your name and mine inside a heart upon a wall
Dm A
Still finds a way to haunt me,

Pretty good job. The only real change I'd suggest is that the "E" at the end of the verses is a "B," but the basic chords are solid. For a little extra, the bass notes could be added under the chords on the verses, e.g., instead of just A-E-G-D-Dm, try A - E/G# - G - D/F# - Dm/F etc.
