The strumming pattern is G:[D DU UPU] Am:[DUDU] Em:[D DU UPU UPU] C:[DUDU]. I'm about 95% certain that's it. Alternatively: G:[D DU UPU] Am:[DUDU] Em:[D DU UPU] Em7:[UPU] C:[DUDU]. Notice, I've added an extra Em7. In a live version (KEXP; Tiny desk), when she transitions from Em to C, she lifts up everything but her middle finger which stays on the 2nd fret of the A string. She plays a UDU before switching to the C. She doesn't do this every time, though. Also, sometimes she plays the C/G without her middle finger.
