One slight correction...
In the chorus instead of G/Gsus4/G it's actually G/C/G.
I watched it live and was abot 5 feet from Chris...

Great version. Thank-you Pwnage.
Along with this transcription I have watched every video of him playing this I can find. A couple of them, especially the one of him in Ireland show his fingers clearly and I picked up a few playing tips that I would like to share. I hope they make sense.
1. At the end of the verse lines when it goes from C to an E and back to Am it's a walk down. Play the C chord, then play 2nd fret on the A string with the D open, then back to Am with emphasis on the open A string.
2. The Em's in the last two lines of each verse are E majors. He clearly frets the G string in the videos and Em doesn't sound right when playing along with him.
3. In the chorus play a regular C chord then drop your pinky on the third fret of the D string to make the sus 4th. It's not really a hammer on and pull off since it's strummed with emphasis.
4. When going from the G to the D in the chorus walk up to it by playing an open A string, 3rd fret on the A string, then the open D string.
5. The end of the bridge is insanely difficult vocally for mortals. If you cannot hit that high resolution note you can resolve it by dropping in an E major chord before going into the solo. It punctuates it nicely.
6. This is in 6/8 time. Strumming pattern is 1 2& 3& 4 5& 6&.Emphasis on 1 and 4.
